LICENSING

The Sabah Pharmacy Enforcement Branch issues Poisons licenses & permit to individual applicants who deals in retail, wholesale or usage of poison as listed under the First Schedule of the Poison Act 1952 (Revised-1989)

Type B License	bform
Sodium Hydroxide Permit	cform

Client Charter

Poisons License/Permit shall be issued to applicants who comply with all the necessary requirements within 21 working days

Issuing License Requirements

	Complete Application Form
	Poisons License Fee Paid (Type B-RM100, Sodium Hydroxide Permit-RM20)
	Applicant comply with the relevant law requirement and fulfill the criteria under the law.
